Prudential Health Care is seeking a Part Time Licensed Vocational Nurse (LVN) Home Health Case manager to join our team. Our office in Cerritos provides Home Health professionals for the local area including Long Beach, Torrance, Los Alamitos, Lakewood, San Pedro, Palos Verdes, Anaheim, Irvine, Tustin, and many others. LVN Home Health Case Manager Job Functions/Responsibilities

• Coordinates home care services for patients assists with the establishment of the plan of care in coordination with the physician and the DON.
• Keeps live contact with the doctors and makes sure that all the updated orders are submitted to doctors in a timely manner.
• Works with Clinical Supervisors/Nursing Supervisors and staff clinicians to gather, collate, and report patient care summary information to Field Staff.
• Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted and services are delivered in accordance with contract provisions.
• Establishes and maintains positive working relationships with current and potential customers.
• Promptly responds to Heralds Home Health customers’ requests and concerns.
• Assists in building a high-quality provider of services.
• Maintains comprehensive working knowledge in the field and shares information with appropriate organization personnel.

LVN Home Health Case Manager Qualifications

• Graduation from an accredited LVN school coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.
• Prior experience practicing as a Licensed Vocational Nurse in the field of Home Health Care.
• Current Valid BLS Certification
• Valid California driver’s license with good driving record.
